PHYSICALS

Click here to access the state mandated physical form.

Every student is required to have an updated physical on file each year.  This must be done BEFORE August 1 and should, if at all possible, not interfere with our scheduled rehearsals. If you have already completed a physical for another school activity (i.e. Athletics), you do not need an additional physical.  Simply forward me a copy of the form.  

If you have any issues scheduling a time for a physical, please let me know.  If you cannot schedule with your PCP, physicals can often be done at local clinics, emergency care facilities, etc.  While we do not currently have plans to offer physicals at school, I will be happy to help you if needed.
